“Clinic to Community is a referral program for individuals with a diagnosis of epilepsy and their families.”

The goals of the program are:

– to provide individuals with epilepsy and their family with information about seizures, seizure first aid and epilepsy and

– to provide you and your family with a network of support services to help everyone in the family to live well with a diagnosis of epilepsy.

Through doing this, we try to reduce the stress of epilepsy for everyone involved, including your family, your health care team, and the Ontario economy because when you have more information you are able to make better use of services and to know how to respond to seizures.

The Four Components of the Clinic to Community Program:

  1. Individualized appointments with a Clinic to Community Educator who is specially trained to support individuals and families living with epilepsy.
  2. The Clinic to Community Educator will provide an overview of seizures, epilepsy, risk management including SUDEP and seizure first aid to the entire family.
  3. A network of services including support groups, peer to peer linkages, recreation programs, and school and workplace assistance.
  4. A series of information sheets filled with strategies to help the person with epilepsy to best manage their condition.

There are 5 modules to this education:

  • What is Epilepsy?
  • Seizure First Aid
  • Treatment
  • Living Well with Epilepsy – Pediatric / Adult
  • Famous People With Epilepsy?

After each module there will be questions which test your knowledge of the modules.
